The answer to the last question is it's really really cheap. You just need a camera, a laptop, AfterEffects and Logic.

The answer to the first question is that it depends. Sometimes there's a lot of shooting to find a good cluster, sometimes you luck out. Obviously compositing it in AE is a bit like needlepoint! Quite time-consuming. But it pays off.
